Children's Hope Counseling Open House! 9/30/11

Open House will be from 10:00 am to 12:00 noon and again from
4:00 pm to 7:00 pm.

New Clients - To save time please print the intake form and have it filled out before your first scheduled appointment. This will save you time and allow you to utilize your entire counseling session. Also, giving us your insurance information before your appointment allows us to verify your copay before you come.
